Vitamin B12 is most commonly associated with energy and neurological function, but its relationship to testosterone is underappreciated and clinically relevant. Men with suboptimal B12 levels frequently present with fatigue, mood changes, and reduced libido: symptoms that overlap significantly with low testosterone. Whether the connection is direct or indirect, understanding it gives men better tools to address the root causes of how they feel rather than chasing a single number on a lab panel.
What Is Vitamin B12?
Vitamin B12 (cobalamin) is a water-soluble vitamin that plays essential roles in red blood cell formation, neurological function, and DNA synthesis. ( 1 ) It is found primarily in animal products including meat, fish, dairy, and eggs. Unlike most vitamins, B12 requires a specific carrier protein called intrinsic factor, produced by the stomach lining, to be absorbed in the small intestine. ( 2 ) The NIH Office of Dietary Supplements provides detailed guidance on B12 requirements and deficiency risk factors.
Deficiency is more common than most men realize. A study published in the American Journal of Clinical Nutrition estimated that up to 6% of adults under 60 in developed countries have frank B12 deficiency, with up to 20% showing low-to-normal levels that may still impair function. ( 3 ) Older men, men on long-term metformin, and men using proton pump inhibitors are at elevated risk due to impaired absorption.
How B12 Relates to Testosterone
The connection between B12 and testosterone is not a direct hormonal relationship but rather a set of converging physiological pathways. First, B12 is required for the methylation cycle, which governs homocysteine metabolism. ( 4 ) Elevated homocysteine, a consequence of B12 deficiency, has been associated with reduced Leydig cell function: the testicular cells responsible for testosterone production. ( 5 )
Second, B12 supports mitochondrial function in Leydig cells. Testosterone synthesis is an energy-intensive process; compromised mitochondrial efficiency in these cells can reduce output even when the hypothalamic-pituitary-gonadal axis is otherwise intact. ( 6 ) Third, B12 deficiency contributes to anemia, which reduces oxygen delivery to tissues including the testes, further impairing their functional capacity.
B12 and Stress Hormones
B12 also plays a role in regulating the adrenal axis. Cortisol, the primary stress hormone, is catabolic to testosterone: elevated cortisol chronically suppresses gonadotropin-releasing hormone and LH, reducing testosterone production. ( 7 ) B12 supports adrenal health and the synthesis of neurotransmitters that modulate stress response. Deficiency can therefore contribute to a stress-hormone environment that is hostile to testosterone maintenance.
What the Research Shows
A study published in Urology found that men with unexplained infertility and low sperm motility had significantly lower B12 levels compared to fertile controls, and that B12 supplementation improved sperm parameters. ( 8 ) Sperm motility and testosterone production share overlapping support systems in the testes, making this finding directionally relevant to the broader testosterone question even if the study did not measure T directly.
Research published in the Journal of Nutritional Science and Vitaminology demonstrated that B12 supplementation in deficient men improved fatigue scores and neurological symptoms, with secondary improvements in mood and motivation: factors that significantly affect subjective energy and drive, even if serum testosterone does not shift dramatically. ( 9 )
Understanding the full hormonal picture matters. Men who suspect low testosterone based on symptoms should review what low testosterone actually looks like before attributing all symptoms to a single deficiency. B12 and testosterone can both be low simultaneously, and correcting B12 alone may not be sufficient if hypogonadism is also present.
Common Misconceptions
B12 Injections Directly Boost Testosterone
B12 supplementation does not directly raise testosterone in men with adequate B12 levels. The mechanism is indirect and permissive: correcting a deficiency removes a constraint on testosterone production and hormonal signaling, but it does not add testosterone to the system the way TRT does. Expecting B12 injections to function as a testosterone booster will lead to disappointment. ( 10 )
Oral B12 Is Always Sufficient
For men with absorption issues, including those with low intrinsic factor, gastric bypass history, or significant gut inflammation, oral B12 supplementation may be inadequate regardless of dose. Intramuscular or subcutaneous B12 injections bypass the absorption deficit entirely. Serum B12 alone may not reflect tissue availability; methylmalonic acid and homocysteine levels provide a more complete picture of functional B12 status. Normal Lab Values Mean Adequate B12
Standard lab reference ranges for B12 are wide, and the lower end of normal may still be functionally suboptimal for some men. Research from Neurology suggested that neurological symptoms of B12 insufficiency can occur at levels well within the conventional normal range. ( 12 ) If symptoms persist despite a technically normal B12, functional B12 markers should be assessed.
Who Should Consider B12 Evaluation?
Any man presenting with unexplained fatigue, brain fog, mood disruption, or reduced libido should have B12 status assessed as part of a comprehensive workup. This is particularly true for men over 50, vegetarians and vegans, men on long-term acid-reducing medications, and those with a history of gastrointestinal surgery or inflammatory bowel disease.
B12 evaluation should sit alongside testosterone, thyroid, cortisol, and metabolic markers in a complete panel. It is rarely the only factor, but it is frequently overlooked. Men already on testosterone replacement therapy who are not experiencing the expected improvements in energy and mood should also have B12 and related cofactors checked, as deficiency can blunt the response to TRT. The same applies to men comparing TRT to natural testosterone approaches: foundational nutritional status affects the outcome of both.
Next Steps
If you have not had a comprehensive blood panel that includes B12, methylmalonic acid, and homocysteine alongside your testosterone and metabolic markers, that is a reasonable starting point. A clinician can interpret the full picture and determine whether B12 repletion, hormonal intervention, or both are indicated. Do not treat symptoms in isolation when the data to understand root cause is accessible with a simple blood draw.
